Kim Fields very briefly went by Kim Fields Freeman when she was married; sadly her marriage ended under not very pleasant circumstances and sent her into a nearly year long depression.

This is kind of a dumb statement, but I don't think that Cheryl Ladd was that "well known" or a household name when she was still professionally known by her maiden name. Kind of like how Demi Moore's maiden/given name is Demetria Guynes but was already married to her first husband, Freddie Moore when she became famous/broke into show business. When she got divorce and subsequently married Bruce Willis, I'm assume that since she was already well known as "Demi Moore", she kept it that way professionally.
