Yes, Uncle Leo was Helen's sister. I don't think they ever said what her Maiden name, or Uncle Leo's last name was.

Yes, Jerry introduced Elaine to his cousin Artie Le-veen. And Artie corrected Jerry and said his last name was Le-Vine.

I have an alternate take on "The Package". After hearing Newman's reacation to seeing who signed for the package and seeing//saying Uncle Leo, I thought it would be very weird for Leo to identify himself as Uncle to the outside world, especially since his pride and joy seems to be his son Jeffrey rather than his nephew Jerry. So my thought is that Newman sees whatever name Leo signs, such as Leo Dingleberry, and just translates it into the name everyone knows Leo by, Uncle Leo.
This of course assumes that Newman would know Leo's actual last name when seeing it on the form.
