I have one from Charlotte Rae and one from Nancy McKeon....
I don't remember the Nancy McKeon address I used, but the Charlotte Rae one is

Go to http://lisawhelchel.com/shop1.html, scroll down to the autograph photo section, indicate quantity 1 and click the "add" button to add the item to your shopping cart. I don't recall what comes next, but you're probably going to be asked for your credit card information and address.
