Was it ever stated why Elizabeth passed away?If so,I don't remember it and I've seen the episodes a lot.

I think I have seen all the episodes, and dont ever remember seeing a picture or hearing of how she died.

In "Home Sweet Home for the Aged" a picture of her was shown on Fred's dresser but not up close.

Actually there was an episode where Tina Andrews played Esther's niece, and Esther said she was a dead ringer for Elizabeth. (I think it aired during the next-to-last season of S&S; Tina was also featured in the horrible "Sanford Arms" spinoff, but, cute as she is, I don't hold that against her!)
