I read in the episode guide that CBS refused to air this two-part episode because of the seriousness of the episode. My question is if CBS did not air the episode, how did it caused major controversy if the viewing public didn't see the episode air on CBS in 1972?

They must have had test audiences or critics that were instructed to watch the two episodes and the feedback was probably too controversial, or too serious. So I'm guessing because of that, they decided not to air the two-parter episode.

The episode did air, on November 14th & the 21st. I believe I read that they couldn't tape part two until CBS approved the first part/it was tested before an audience. Many CBS affiliates refused to air it, but overall I believe it was aired on most CBS stations.
