How do they do that??

Guessing, but I don't think those are her real hands, the arms are at work inside that loose fitting smock, which is likely fitted around a wire frame attached to her elbows.

Notice the lurch when she pulls her shoulders out of the top of the garment while she slides her neck down the open front of the garment....that's when I think the frame becomes supported by braces attached to her elbows.

You can definitely see her knees and thighs under the garment as she stands up, she is in a hunched position while the head protrudes from her mid section.

So, supporting the hollow torso while she is hunched over is the trick, and The way she can lay so flat to the ground is pretty amazing....
