The Official Dick Van Dyke Show Book lists only the last names. It lists the song as writen by David & Livingston. I hope this helps you at least get started in the right direction.

No problem TJ. I'd like to think that I know just about everything there is to know about The DVD Show. It's my favorite, and I have every book published about it. I wish there were more people around to talk about it.
