Clown on Newhart couch - Sitcoms Online Message Boards

I have found a image via google numerous times of a clown on Dr. Hartley's couch but cannot tell if this is a photoshopped joke. Does an episode exist of "The Bob Newhart Show" with a clown? It does seem familiar to me. I have been googling for hours to find out.

I do remember that episode- it was just a short scene that begins with Bob saying something like "You have to realize people may not take you seriously in your work all the time, but you just have to keep smiling through it"- and when the camera pulls back it is a clown laying on his couch. Don't remember which episode it was, though, as it was just a short part that had little to do with the actual storyline.
