No, I don't think he looks annoyed. Maybe happy he's leaving?

I'm surprised that Tom was willing to be the last man for the credits for the whole show. I would have thought he would have wanted to be right after Marion.

Some shows choose to do that - have one of the biggest stars last in the credits. Rip Torn in The Larry Sanders Show and Joseph Kearns in Dennis the Menace come to mind, but I'm sure there were others.

He was the only one to get his real name AND charcter name in the opening credits with the And Starring Tom Bosley As Howard Cunningham. Or is it also starring instead?

It was both! "Also" was used in the first two seasons and "and" for the remainder of the series.

He wasn't the only cast member to have both his name and character's name in the opening credits. Marion Ross had hers starting with the tenth season.

I don't think Don looked mad. He kind of looked to me like he was half way smiling and half way trying not to laugh at something.

I'd bet trying not to laugh. remember the previous season's opening credits, he had this big laugh and threw his head back. I'll bet when they filmed the cast shots for the openings, they were trying to make them laugh and smile. Maybe he didn't like his shot from the previous years with the big laugh and tired to just keep a smile this time.
