I never recall anyone mentioning her...how she died...when she died...did she die??? Nothing was ever said even in passing...did I miss something?

Yes, she died. Very little is said though; I don't believe any details were provided at all. Aside from the very first episode, where there's some reference, it's just a given that Andy is a widower.

I only recall once, maybe twice any reference to Opie's mother whatsoever. The implication is that she died when Opie was very young. I never heard any reason as to how she died, or exactly when, how long they were married, or even her name.
