As for my answer, Denzel Washington in Fallen, Man on Fire and Training Day. I believe he died in Glory as well.

He was killed off in the following movies:

Culpepper Cattle Co.

On TV, he was killed off in episodes of Gunsmoke (twice!), All in the Family, Deep Space Nine, and of course he played Lt. Rodriguez, the ill-fated cop on Miami Vice.
