Mine was from ep #114, Season 4, "Birdies, Bogies & Baxter", to improve Darrin's golf game...

Endora:

"Eye of newt & scale of fish,
Give this mortal his stupid wish!
Let those clubs that he did aim,
Give him back his college game!"

Then Endora cackles and pops out.

"bees knees and bells nells return this mortal to his former state, if this doesn't work I simply can't wait".....Endora pops out, I don't remember which episode it's from, but it's my favorite.

Stickem stagnum buck
B-U-C-K spells buck and out goes YOU!"

Another one Sam used to bring Endora back (in 'Eye of the beholder" Season 1)

"Quadramus invecta expedia!"
