What happened to Sylvia Sidney as Mamma Carlton?

Sylvia Sidney is dead now, but she only appeared once as Mama Carlson and that was in the pilot episode. Carol Bruce took the role later in the first season. I don't know why Sylvia Sidney left the role. My guess would be that she was primarily a film actress and maybe had a prior commitment to that. Just a guess.....

I think Sylvia Sidney played her again in at least 1 other episode.

No, Sylvia Sidney only appeared once as Mama Carlson and that was in the poilot. Every other time Mama appeared, Carol Bruce played her. Speaking of Carol Bruce, I just saw her in a Charlie's Angels episode from their 4th season entitled "Three For the Money".
