I Use Womble's MPEG-VCR Mpeg2 editor. Works great. TMPGEnc also makes an great Mpeg2 editor as well.

I used to use Womble as well, but have since switched to VideoReDo which I love!

There is also a very easy way to do it using DVD shrink or Nero Recode (both software applications are written by the same author), but it has one tiny drawback to it, which is that it creates a separate title for each segment, so if you wanted to be absolutely "tidy" and "obesessive" about it you would have to burn to DVD, then run it through DVD shrink (or Nero Recode; which is the same basic application) one MORE time to create one single title..but it works great either way, because all DVD shrinks video discs play all their titles non-stop, there is NEVER a main menu, unless you has intentionally have DVD shrink add the one you have.
