I think Tony Danza had a short-lived show during the late '90s in which he played Tony Danza.

Larry David plays himself on 'Curb Your Enthusiasm'. His wife Cheryl is played by Cheryl Hines, and his friend Jeff Greene is played by Jeff Garlin.

On 'The Cosby Show', Peter is played by Peter Costa.

Ron Glass played Detective Ron Harris on 'Barney Miller'.

Paul Reiser played Paul Buchman on 'Mad About You'.

Funny thing about Will Smith...on Fresh Prince, he was WILLIAM Smith, but in real life, Will's short for WILLARD.

Keeping on topic...Bernie Mac as Bernie Mac on the self-titled Fox sitcom. Mac (in real life and on the show) is short for MacCullough.

Also, Martin Lawrence as Martin Payne and Tommy Ford as Tommy Strong (or Strawn, according to some) on Martin.

For one-time appearances, how about Ken Berry, who played Ken Kelly on The Brady Bunch as a guest star in #107 "Kelly's Kids"? Granted, there was talk of this being the basis of a spinoff.
