The Flintstones episode 1.14 "The Prowler" (1960) has been removed from reruns and streaming because of a stereotypical Japanese character. It had already been released on DVD.

The most famous case involved the original Hawaii Five-0 episode 2.16 "Bored She Hung Herself" (1970) wherein a character accidentally hangs himself while meditating. A home viewer decided to try it and died. After the smoke cleared with lawyers and lawsuits it was determined the episode would never be seen again. To this day CBS has not released the episode in reruns, network or local broadcasts, in streaming or on DVD including what is called The Complete Series. A low-quality copy can be found online.

I read somewhere that there had been discussions in the United Kingdom about banning a “Fawlty Towers” episode, entitled, “The Germans.”

That 1970s FT episode features some decidedly “frank” racial epithets. FT creator and star John Cleese was not happy at all about the possible banning of that episode on British television.
