That never made sense to me.

It's not exactly the same, but on Gilmore Girls the credits always say "Special Appearance by Edward Hermann"... which, similarly, I never understood, because he is just as much of a "regular" as any of the other actors that they listed during the opening...

Heather Locklear was technically the star of Melrose Place but she was always billed as a Special Guest Star in the opening credits.

Billing is something that is usually negotiated as part of contract negotiations. This usually includes not only how the actor wants to be billed but also where (first billing, last billing etc.). I assume that this is how Edward Hermann requested to be billed.

She appeared in 34 out of 168 episodes, so she was hardly a regular which is probably why she received the special credit. On "Terminator: The Sarah Connor Chronicles," Richard T. Jones always received the "And" credit and a few of the semi-regulars received the with credit--- Brian Austin Green in the first season before he became a regular as well as Dean Winters, who also received the "With" credit in the second season--- Stephanie Jacobsen and Busy Phillips also received the "With" credit in their season two episodes. On Frasier, John Mahoney got the "And" credit. Other notable mentions:

For some reason, Edgar Buchanan received the "With" credit on Petticoat Junction right up until he took over the starring role of the show when Bea Benaderet passed away.
Another odd billing matter: for the first season of the Munsters, Fred Gwynne received the starring credit but was billed last.
I've often wondered the significance of these "special credits" and the criteria the showrunners have of who gets what billing. This kind of stuff fascinates me. My theory is that whoever gets the "And" or "With" credit is usually not the protagonist of the show or film at hand, and sometimes not very integral to the overall plots at all--- but they usually provide some kind of comic relief or add something special to the show that can really only be delivered through that specific character. Let's take Sophia Petrillo as an example; not quite the main protagonist of TGG, and the show could still have been successful without her, but she added something to the show that just made it stand out even more--- something special. This may not make any sense, but it's just a theory LOL.
I'm sure there's hundreds more...

He continued with that billing for the second season and the few episodes of season three he was in but by that point they stopped having the credits in the theme song and they just appeared on the screen in the early part of the actual episode.
Also, "Community" had "and Chevy Chase". It didn't list "as Pierce Hawthorne" but it was an "and" credit, probably since he was the biggest name attached to the project at the time. All the actors have since gone on to be quite busy.
