There are probably more, but ALF and Cybill comes to mind.. both shows ended their last season with a cliffhanger. While ALF had that awful TV movie, the Cybill cliffhanger was never resolved.
My Name Is Earl ended with a cliffhanger but I'm not sure whether they had the onscreen disclaimer.
