Yes, she was a ventriloquist. As a child, she was very quiet and shy. Her parents ordered her the dummy from a Sears catalogue, and she started ventriloquism (is that even a word ?!)

I liked the episode in one of the later season where the ventriliquist comes to Langley and Blair gets her dummy, Jo-Jo. That was funny. http://www.sitcomsonline.com/ubb/smile.gif

She did some in Boy About The House with the Little JO doll, remember that, when Andy was being adopted? And Blair just wanted her picture to be retaken?
