In the episode "Committeeman Fred" ...at the end Fred says as a reward for turning in Sam Jordan for bribary...he asks for the key to the city and a lock on Ms. Chandler's mouth. Her correct name is Mrs. Channing. He even calls her this in the beginning of the episode.

Sounds like a mistake. Maybe it was to show the character was senile.
