This brief presentation is based on medical articles from the SPINE journal (www.spinejournal.com) and supported by my own symptoms and observations.

Adult lumbar scoliosis is commonly due to two different causes. The first is a person who had scoliosis as a child and this curve has increased into adult life or is becoming painful. The second type of adult de novo spinal deformity is seen in patients who have never had a scoliosis but begin to develop curvature and pain with aging.

Pain is relatively minor consideration in the treatment of adolescent idiopathic scoliosis, but there are a group of adult patients with scoliosis (see my curve bellow, measured by Cobb method) who present to physicians with the chief complaint of chronic and debilitating pain. The curvature of the spine is mostly due to degeneration (DDD) of the intervertebral discs. DDD (Degenerative Disc Disease) may start after disc's injury or due to aging. Unevenly tilted and bulging disc (because of post injury tear and/or dehydration) will become the foundation for spinal deformity. Though not commonly recognized, this deformity can progress in a slow, insidious, lasting years evolution, after non-fusion surgery as well. It will affect not only the anatomic configuration of the spine but also spinal function and mobility. Sitting and bending will become difficult, uncomfortable and/or painful. Upper back may slowly develop pain and discomfort when the thoracic part of the spine will swirl gradually in the opposite direction forming compensation curve.

In addition to back pain, adult patients are more likely to experience symptoms of spinal stenosis and radiculopathy from rotary sublaxation, which may lead to nerve root impingement and stretching. In other people the spine looses its stability which can be painful and lead to symptoms in the legs such as numbness, tingling, burning, parasthesia (over sensitivity) and weakness. This is due to a pinching or pulling of the nerves as they pass through the spine to the legs. All nerves of the cauda equina (from L1 to S5) may be affected as they travel along the vertically curved/horizontally twisted path in the spinal canal. Most symptomatic (to find out where on your body, just look at dermatomes picture to the left) are however distal nerves ends, giving parasthesia and pain; the longer the root is, the more disturbing symptoms are at its end (pain under the soles from L5,S1 and S2 is a classic example).
