He's always had a kidney problem- that is why he is the size that he is.
He's had quite a few major operations in his life but I don't actually know if he is especially in good or bad health at the moment.

Gary Coleman suffers from a congenital kidney disease causing focal segmental glomerulosclerosis (an autoimmune destruction and alteration of the kidney), which halted his growth at an early age. He underwent two kidney transplants, one in 1973 (before Diff'rent Strokes premiered) and one in 1984 (during the series's 7th season), and requires daily dialysis.

Poor guy.
