My Favorite example of this is The Mother’s In-Law. 5 out of the 6 main characters were named after the actor who played them until Richard Deacon took over for Roger C. Carmel. Deacon's character name stayed Roger. I guess no point in confusing people anymore concerning Carmel and Deacon did not at all resemble each other.

Going way back, many non-regular characters on The Adventures of Ozzie and Harriet, particularly kids and fraternity members, used their real first names in the show (the Nelsons used their full real names).

Gee, how could I have forgotten about that? The first sightings of Stanley and Barry Livingston was there with character names to match.
