The Buffyverse Curse - Other than Alyson Hannigan and David Boreanaz, acting on the series Buffy: The Vampire Slayer didn’t do much for most of its performers’ careers, including Trachtenberg.
Bad management - Given her looks and her middling acting ability, she should have landed in higher profile television roles than she did, post-Buffy. She didn’t, and you really have to chalk that up to bad management.
She seems to have lost out on the Bella role in the Twilight franchise - Had she received that role, her career would have gone in far different directions. She didn’t….and it didn’t.
She’s never grown better as a performer - She probably should have done some stage work, maybe some arthouse films, and even tried sketch comedy. Basically she’s nearly 40, and an average performer in a Hollywood replete with just such people.
She clearly has one or more health-related issues - While Trachtenberg has been lashing out angrily, even people with no medical training can perceive that she’s ill and has been for at least the last five years. It’s changed her appearance radically; and since she was cruising off of that, things have not been easy on her. Hopefully she’ll get her health issues under control (if that’s possible) and she’ll be able to get her career back on track.
