For example, Lisa Nicole Carson essentially got dumped from Ally McBeal because she was bi-polar. Kristy McNichol left Empty Nest to concentrate on her mental well-being, only returning for the series finale. Michael O'Hare had to leave Babylon 5 after the first season because he started having psychotic symptoms (which turned out to be schizophrenic) that were bad enough for everyone else to take notice of.

I was going to mention Michael O'Hare if you hadn't. I just recently started watching Babylon 5 on Amazon and I'm mostly enjoying it. Just started the second season a couple days ago where Bruce Boxleitner replaced O'Hare. The stories are mostly good and the acting's great but I think a lot of the special effects are sub-standard. I looked up a lot of the actors online so that's how I found out about O'Hare's mental illness beginning in the first season and that it caused him to have to leave the show.

I believe Grace Under Fire ended in part, due to Brett Butler's erractic behavior on set. I also wanna say that the parents of the actor who originally played her son pulled him from the show as a result.

Yes. Apparently, Brett was drunk and flashed in front of the boy who originally played Quentin. That and her behavior resulted in the character being aged up and replaced by a teenager.
