I am sorry that I can't comment on seasons 6-8, but the very first episode of the series has something that has always bugged me:

Alex mentioned he gave Eddie Barkley a placebo. Donna said "what's that?"

If Donna was a nurse and she was married to a doctor for 14 years, why didn't she know what a placebo was?

I realize Donna asked that question for the benefit of the viewers, but it made her sound like an idiot. Instead they could have had her say something like "oh, you mean those harmless sugar pills?"

And Alex didn't even react when he had to explain it to her!

They probably hadn't fleshed out the character enough, to even establish that she had been a nurse.

If they had, they could have had Jeff there (I don't remember the scene to know if he was), and he could have asked the question.

Yes, even a doctor's wife who hadn't been a nurse would probably pick up that term in 14 years.
