Weird. Why would a boss not call him Mr. Evans or James?

How come a mentally "sick"/disabled person can't call his supervisor or boss or school-teacher by his/her surname rather than his first name (like Mr./Ms. [surname])? I understand that at a professional setting, you always had to refer to your boss/supervisor as Mr./Ms. [surname].
