After RGIII got surgery on his leg, his doctor gave some medicine that should prevent it from getting more damaged.

But here's the problem, still after the surgery RGIII was somehow imbalanced. It seems like his doctor may have given a wrong medicine. The wrong medicine, not only supposed to cure his leg, it also went into RGIII's brain and affected it too much.
