I mean REAL feuds, not like the semi-friendly rivalry between the Yankees/Red Sox and their respective fans, or likewise the Browns/Steelers. How about teams and/or individual players/coaches/managers/officials, etc. who cannot stand each other?

I have a few:

The Chicago Bulls and the Miami Heat (NBA)

Zack Greinke and Carlos Quentin (MLB)

Omar Vizquel and Jose Mesa (MLB)

John Rocker and...just about everyone (MLB)

Ozzie Guillen and...just about everyone (MLB)

Next?

Yankees/Red Sox semi-friendly? Now that's funny. That's like saying Barack Obama and Osama Bin Laden were friendly. These two teams, and by extension, the cities they play in (New York and  Boston  ) have hated each other for over 90 years ever since Babe Ruth was traded by the Yankees to the Sox. The other names you mentioned, well their rivalries will be forgotten in about a week, at least by me. For real rivalries, try these out:

Baseball

Yankees/Red Sox
Dodgers/Giants

NFL

Cowboys/Redskins
Bears/Packers

NBA

Celtics/Knicks
Celtics/Lakers
Heat/everyone else

NHL

Maple Leafs/Canadiens
Maple Leafs/Senators
Rangers/Islanders/Devils vs each other
Rangers/Flyers
Red Wings/BlackHawks
Penguins/Flyers
Oilers/Flames

+ all "original six" teams vs each other (Toronto/Montreal/Rangers/Boston/Chicago/Detroit)

I'm sure the rest of you can come up with a lot more.

Well, I said "semi-friendly" because the feud doesn't seem to be personal. What I meant was, people in pro sports who have personal vendettas against each other. With the Bulls and the Heat, the players have nothing but bad things to say about the other team, and one of the Bulls intentionally shoved LeBron James to the floor (which I have to admit I didn't shed any tears over.).

Wrong again! Just ask any of their fans!

I'll take your word for it, since I'm not from either of those cities.

Individual feuds among former pro athletes and broadcasters:

Tim McCarver vs. Deion Sanders
Harry Caray vs. Milo Hamilton
Lee Elia (former Cubs manager) vs. Wrigley Field fans -- most intense, expletive-laden tirade (or rant) from Elia in pro sports history

^ What are the stories behind those feuds?

My memories are not as good as they should be: McCarver (in the broadcast booth) made some less-than-flattering comments about Deion, when Sanders had an MLB career going. IIRC, McCarver ended up getting a Gatorade shower, or something like that.

Milo I think believed he was set to get the main Cubs broadcasting role following Jack Brickhouse, and then resented Caray getting in there and taking it (Caray had been the Chi. White Sox voice previously).*

Elia's Cubs were not doing too well (big surprise -- not) and he let the fans have it (mostly the "bleacher bums") after getting tired of their shouts of criticism (come out to this blankin' park every blankin' day...). Sometimes the rant (with bleeps) was played heavily on Chicago radio stations.

*Edit: I've checked and found that as I recalled, Hamilton stayed on for a while after Caray was hired, but then blamed Caray when eventually WGN fired him.
