In every other season of Family Affair it included a page with the episode listing. Did MPI not include one for the fourth, or am I just unlucky and they didn't include one with my set?

My season 4 set didn't have the episode sheet either. If I recall, the same thing happened with season 5. I contacted MPI, they were very kind and sent me the episode sheets. Perhaps they can do the same for you.

I didn't receive one in the original Season Four set. They did include an episode listing in the Complete Series boxed set. The episode listings were printed inside the cases on the artwork and not on sheets. They used slightly different backgrounds and fonts.
