There was already a Michael Fox registered in the Screen Actors' Guild, so he decided to add a "J" as his makeshift middle initial. Why "J?" Because it sounds better to him than "Michael A. Fox," which would better represent his real middle name - Andrew. As for what the "P" stands for in "Alex P. Keaton" - I don't think that was ever established.
