for those that remember a short-lived sitcom from the 70s called "HOT L BALTIMORE" (I think it was from Norman Lear. It was set at a hotel called the Hotel Baltimore; but the "E" was burned out on the neon sign)
Anyway, there was a character who was ocasionally refered to but never seen called "Moose" Valoti; or something like that.

Pete's wife, Gladys, on December Bride. We finally did see her when the show was spun off as Pete and Gladys but she never appeared on DB.

For years on Happy Days, Jenny Piccolo was referred to but wasn't seen until much later in the run when they cast Cathy Silvers.

Yes she was in a season 9 episode where Fonzie has to goad Chachi into going to see her.

Two other characters we heard about but never saw:

Magnum PI : Robin Masters. It was actually revealed in the finale that Masters was, in fact, Higgins!
Charlie's Angels : Charlie himself. We did hear his voice (by John Forsythe) but never saw him.

The Principal - Welcome Back, Kotter (I think he appeared later on though).

Principal Buttsavage - Doug (Nick Version)

The PTB (Powers to Be) on WCW during the very early 2000s (until it was much later revealed that the PTB was Vince Russo.)

Travis (Webster's father) - Webster (until they showed a dream sequence episode later in the early seasons)

Six's mom made many appearances on the show. She was played by Gail Edwards (the lady who played Vicky on Full House). The best part about her character was that she spoke with the same speed that Six did and they did it together!!!

"Will" Thornberry from the Adventures of Ozzie and Harriet; the son of the Nelson's next-door neighbors. He is a friend and schoolmate of David.

He is constantly mentioned in first several seasons until the Thornberrys are replaced by the Randolfs as the Nelson's neighbors. As the first season of this show was from 1952, I wonder if this is not the first instance of a character being referred to but not seen.

No, Sheridan did not appear in any of the episodes. The closest was once, when we saw his car leaving. And since he most likely was gay, I doubt he had a girlfriend. Maybe you're thinking about a different Sheridan altogether? I meant Sheridan in "Keeping up appearances", a Brittish sitcom.

On Mr. Belvedere they had neighbors called the Hufnagels who had a bit of a rivalry thing going on because Wesley was always pulling pranks on them

Eventually, we did see one of the Hufnagles in Giselle (played by Teresa Ganzel) on part 1 on of the series finale.

She was also in the episode where Fonzie moves into the apartment about the Cunningham's garage.

Stanley Walker on WIll and Grace

Ugly Naked Guy on Friends

Charlie on Charlie's Angels

There was a fifth season episode episode of Friends that U.N.G. Appeared in. It was the one where Ross moved into UNG's apartment.

The Ditmeiers, the Brady's next door neighbors.

Mr. Dittmeyer was in an episode of T.B.B. It's the one where Carol learns to fish, and the hook flies over the fence and snags Mr. Dittmeyer's toupee.

Steve Urkel mentions his Uncle Cecil a number of times on "Family Matters", but I don't believe he's ever shown.

ALF mentions his cousin Blinky on "ALF", but he is never seen. (And there are other family members and friends mentioned at times that are never seen, presumably because they all perished in the destruction of Melmac. Though obviously Skip and Rhonda survived.)

On The Muppet Show (and many other Muppet productions), Bernie the agent is referred to many times and characters often call him on the telephone (Miss Piggy being one of his most famous and demanding clients), but he is never seen. (Though there is technically Dom Deluise's character of "Bernie the Agent" in "The Muppet Movie", which may count as the same Bernie referenced in all the various Muppet productions. In any case, that's the only appearance for the character- in that movie, but never anywhere else.)

Charlie on CHARLIES ANGELS; they would show the back of his head and we heard his voice but that's all. (John Forsythe did his voice)

This may not be exactly in this category, but I thought it was worth mentioning.
For years, Big Bird referred to his friend Mr. Snuffleupagus on "Sesame Street"- and while the character was shown on the show- he was only seen interacting with Big Bird (and possibly other Muppets) but always somehow vanished right as the adults were coming by to see this friend Big Bird was always telling them about. Sometimes Snuffy would be hidden or sometimes he would have just walked off right before Big Bird and the adults got back to where he was and so forth.
There was an important episode in 1985 that finally had the adults seeing Snuffy and then believing Big Bird and apologizing to him for not believing him before. (This was mainly done because of concerns that children may get the idea that adults wouldn't believe them if they told them about sexual abuse they had received and such, since the adults wouldn't believe Big Bird about Snuffy.)
Of course, Mr. Snuffleupagus since became a regular character that everyone saw and interacted with on the show.
