In Charles In Charge, you see a younger picture of Mario Baio(Scott's father) when Scott is speaking during the episode with the blind girl.
In Punky Brewster, you see a photograph of George Gaynes' real life Allyn Ann McLerie next to the couch

Kellie Martin did a TV movie back in 1995 called "The Face On the Milk Carton", based on Caroline Cooney's best-selling series. There were all sorts of childhood pictures of the girl she played. It was quite obvious that the pictures were of Miss Martin as a child. It couldn't have been anyone else but her.
On the TV show Family there were always family photos/snapshots around the living room, especially atop the piano. They had a couple of black and white glamour shots of the mom, Kate. Those obviously were of her portrayer, Sada Thompson, in her starlet days.
While Michael Moriarty was still playing the EADA on Law & Order in the early 1990s, there was a picture of a child on his desk. It was revealed it was a picture of Mr. Moriarty's real life son Matthew.

Don't know if this counts, but in the opening of Growing Pains, they show pictures of the characters growing up. You can tell that they're real pictures of the cast when they were younger.
Oh, and of course, the picture of Mary's real-life son that was always in Mary Richards' apartment. (I believe she referred to him as a nephew in one episode.)

On Married With Children the kid that the Bundy's adopted disappeared after he didn't work out and his pic was shown on the back of a milk carton as a joke.
I Love Lucy-Lucy is looking through Look magazine and and there is a baby pic of her real life son Desi jr. on the cover.

Wasn't the photo of Mildred on Col. Potter's desk his actual co-star from another show (Pete & Gladys maybe)? I remember seeing something about that on the boards but I can't access the P&G board this morning to check.
