It's epsiode #45, "Where There's Smoke, There's Rhoda" (Season 2). The fire was started by a faulty refrigerator cord.

Phyllis:What did you do, leave the iron plugged in?
Mary:The fireman said the fire was caused by a faulty refrigerator cord.
Rhoda:Yeah. Careless of me, wasn't it, Phyl? To leave my apartment with my refrigerator plugged in!

Was the fire Rhodas fault?

Well, it's never spefically stated in the ep, but since she wasn't even IN her apartment when it caught fire, I would have to say no. Besides, it was prob. Phyllis who put the refrigerator cord in, since she was the landlady. Maybe she did it on purpose. ;) J/k
