Here's something to scratch your head about.

I was wondering, how early in the show does Maddie stop referring to David by the plain and formal "Mr. Addison" and start to be on a first-name basis with him? When did we really see it become the norm for her to walk in, put her briefcase down and say, "Good morning, David"? I remember she didn't really like him or the company for the first few episodes, and I would like to know when the difference in thinking started to set in.

Through out the entire series there were times that Maddie called David : Addison. She never stopped completely.

She does call him David already in the Pilot, but to find out when it's a norm I'll have to watch some more....

I had actually noticed this one before myself. I don't know when it started to be common, but if I remember correctly even after calling him David when they're alone becomes the norm, she continues to call him the more formal "Mr. Addison" in front of the employees. I think the latter continues for some time.
