where Terri is asked to be a roommate...during the party and the "million laughs", Jack accidently calls Terri "Tammy"...I noticed that during its first run.

I remember that. He also called her Sherri in the episode where she was playing the violin. He walked through the door and said Sherri and then said Terri. I can't remember the name of the episode.

I've always thought that he was supposed to scall her Tammy because he was making fun of her at the party and calling her Tammy emphasized how he didn't care about her.

I agree.

I'm sure the director wouldn't allow John to make such a blatant mistake in calling the character by the wrong name. Jack was making fun of her.

He also called her "Teary"...which I am sure in the moment of the action John was doing just mispronounced her character's name there.

Directors don't always "catch" things...did you ever see the movie PRETTY WOMAN??? That was directed by Garry Marshall (Happy Days, Laverne & Shirley, etc) MISTAKES UP THE ASS!!!!! I have seen too many goofs and bloopers on t.v. shows. Sometimes it is never caught...sometimes the editor thinks no one will notice or care. Sometimes "no one" catches it. There are many reasons why mistakes aren't taken care of. There are sooo many websites devoted to bloopers and mistakes for sitcoms.
