I think it's at the part where Jack is about to take the phony glasses off to show off the grease under her eyes.

I've always wonder that too!!! Like it could have been a mistake and no one caught it or it could have been writen like that to show he didn't even care to learn her name right; which would have been a good thing b/c then afterwards they make up and he takes her as his and Janet roomate!!?!?

Yeah I agree with Drum. I think maybe because he didn't like her at first he just totally forgot her name and didn't care. Either that or John forgot, lol.
