The two that come immediately to mind for me were when Henry Blake (MASH) was killed on his way home from Korea, and Bob Newhart waking up from a dream on the series finale of Newhart.

That really was a "WTF?" moment for me. I think it's kind of a cool twist, but corny at the same time. Hopefully they'll make it work.

I was left staring blankly at the screen for quite a while at the end of the Magnum PI episode "Did You See The Sunrise" when he assassinated his former POW camp tormentor, Ivan, in cold blood. Ivan acted smarmly thinking that Thomas would let him go, then Thomas asked, "Did you see the sunrise today?" and BAM! he pulled the trigger and the screen freezes as the fire from the gun's barrell covers the screen.

I know it's 15 years old, but the second I don't say that the whole series will come out on DVD and someone will say, "thanks a lot for ruining the end for me."

Ross's wedding vow mixup on the fourth season finale of "Friends" and Susan's death on "Seinfeld" come to mind.
