John Wayne was on a publicity tour and made a stop in Tuckahoe to visit his number fan Arthur, and when the ceiling caved in at his house, he moved the party to Maude's and that's how he ended up at Maude.

I read somewhere that the reason John Wayne just came inside the door and was standing behind the couch is that Bea Arthur was taller than him and he was on a box to look taller than Maude

Yes, John Wayne did do a cameo on Maude. The episode centered around John Wayne knowing Arthur, and Wayne coming over to Maude's house. Maude in the episode decided to scold John Wayne for his conservatism and was saying how much she detested the man. However, when John Wayne showed up at her door, she was so starstruck that she was all over him.
