I assume she gave birth over the span of "months later" as show in the Season 4 premiere.

I wondered the same thing, and came to the same conclusion. I forget - was Carla pregnant when Sam returned to the bar? If not, maybe she had it while he was abroad... but then why would she return to work so soon?

I was wondering the same thing, I'm at work right now but I'll check later. I think Sam was abroad for only a very short amount of time.

I believe Rhea's children were born in 1983, 1985 & 1987, so she would have been expecting in season 4

She wasn't expecting in season 4. She was in season 3, which ran from 1984-85, thus coinciding with the birth of her second child.
