I don't think this is true, but I read somewhere that she died while napping on the set. I know Redd Foxx died on the set of Royal Family, but as for Zara I think it's an urban legend. She was in her 80s at the time and the year before her passing she only did 2 episodes because her health was poor. In one of them she's in a wheel chair and they said she slipped on the ice. It's really something when a frail senior citizen is on a popular TV show. Their death could occur right there.

I think she had already finished filming and was too ill to continue, so no she didn't die on set.

Zara Cully did look sick during her last days.

Zara Cully died in the hospital with her family and Paul Benedict by her side. Paul Benedict, (Mr. Bentley) also gave the eulogy at Zara Cully's funeral.
