Desperate to get the theme song music

I'm to perform the theme song of the show on Friday (9/23/05). I thought I had the music but found today that I don't. I need for someone to send me the guitar chords for the music properly placed among the lyrics. I think i can pick out the melody on the piano by hear as long as I have the guitar chords.
